First visit
Procedure for initial (first) visits
In principle, a letter of referral (patient referral document) from another medical institution and an appointment for the date and time of consultation are required when visiting this hospital.
First-visit patients should bring a letter of referral.
Patients without an appointment may not see a doctor on the same day except in cases of medical emergency.
On your initial visit, please fill out the required items on the registration form (A4 size white paper) located on the forms counter and submit it to the “initial visit reception" counter along with your health insurance card and referral letter.
This hospital accepts your My Number Card as your medical insurance card.
Identity authentication (myna reception) can be carried out easily by place your My Number Card on the card reader which includes facial recognition installed at the reception counter.
If you do not have referral letter from another hospital at the time of your initial visit, you may be charged 8,800 yen (tax included) as prepayment for the initial medical examination (fee for selective treatment by patient choice), in addition to the first medical examination fee under your health insurance.
